Research Breast Augmentation in Columbus, OhioFor a few days after your breast augmentation surgery, you may experience acute pain. Full recovery from this surgery can take up to six weeks. For a smooth and successful recovery, prpare your home and get ready at your workplace. Here is what youc an do as you prepare for breast augmentation surgery.

Prepare Your Home

At home, you will need a place where you can rest easily without getting up for various things, most importantly your medication and liquids. The best position for you is in a reclining chair with you back elevated. It is much easier to get up from this position than from a bed or couch. On a table next to you, in a ddition to your medication being readily available, have a good suuply of what you would like to drink. On that table, also have what you plan to use to occupy your time during recovery, such as your laptop computor, books, television, remote, etc.

Check that everything is plugged in, the remote controls have batteries, the clock reads the right time, and everything in the area where you will spend time is in proper order.

Your physician may recommend that you use some medications, clothing, or gauze following your surgery. Stock up on all of these items and keep them in easy ready of your recovery area. Keep extra planets and pillows in easy reach of the recovery area, as well. If you have a lap desk, pull it out and pur it nearby.

Doing this is for your convenience. It does not mean that you need to confine yourself to one area. Your will want you up and moving around and moving your legs frequently after your surgery.

Arrange for Childcare

If you have a young child who needs to be lifted regularly (or a pet, for that matter), make arrangements for someone else to lift and care for your child during the early recovery period.

Your physician will be able to give you a rough estimate to help you gauge how long you will need this help, but ask your helper to be available on standby even beyond the period when your physician says you will definetely need it. This way you will be ready if your recovery takes slightly longer than expected. If you have a pet who needs to be taken out for walks, make arrangements for this to happen as well.

Make Arrangements for Personal Help

Even if you do not have any children at home that will need help throughout your recovery, you will still need help with personal care. If you live alone, ask someone to stay with you for the first few days to help with dishes, meals, and anything else you might need. Since you will also need a ride home after the surgery, this may be the person who picks you up from the surgery center.

Pay Bills in Advance

in the first week after your surgery, focus on your recovery. If bills are due around the time of your surgery, pay them in advance or set up an auto-pay function to ensure that the bills will be paid without any effort from you.

Take Time off Work

Many people who have relatively sedentary jobs are able to return to work within a week after breast augmetnation surgery, but recovery times do vary. Those with more strenuous jobs will need a longer breast before returning to work. Alert your employer well in advance of your surgery to ensure that you will be able to take off this time. Let your employer know that your return to work will depend on your surgery, so you may need some flexibility with your return.

If possible, schedule your procedure for a time when you are sure work will be slow. This will help ensure that you will feel comfortable being at home during this time.

Get Chores Out of the Way

In the days prior to your surgery, get hosehold chores our of the way with the knowledge that it may be several days before you are able to do any of these chores again. Do the laundry, clean the house, do all the dishes, and take out the trash.

As you are doing these chores, think ahead to what might become relevant during your surgery. Is the dishwasher working? Do you have enough dish soap? Will the trash need to be taken to the curb while you are in recovery, and if so, who will do it?

Check all of your cleaning supplies and buy any that you will need. While you are out, think ahead to the meals that you will be eating during your recovery. Microwave meals and soups in a can area n excellent and easy food selection for your surgical recovery.

Have a Consultation With Your Physician

In the weeks before your surgery, have a discussion with your physician about what to expect during surgery and recovery. You will probably have a variety of questions to ask about your recovery and the surgery itself.

Write down your questions so your conversation with the physician will be as productive as possible. Bring a notepad and a pen with you for the talk, so you ccan write down answers and a list of reminders or things to do as you think of them. Get contact information for your physician, so you can send your doctor any other questions that you might think of before the day of the surgery.

Breast Augmentation in Columbus OhioALCL is a relatively rare form of lymphoma and makes up only about 1% on non-Hodgkin’s lymphomas. It has recently been reported developing adjacent to certain breast implants in women who have had their implants for an average of 8 years. The most common symptoms include swelling of the breast due to fluid accumulation in the pocket around the implant or a lump found in the breast next to the implant. You should consult your surgeon if you develop any of the above symptoms.

Textured breast implants almost exclusively have been implicated in the development of BIA-ALCL and the vast majority of cases have been in women with ‘macro-textured’ implants such as those produced by Allergan rather than ‘micro-textured’ implants made by Mentor. The majority of implants used today are smooth walled prostheses, which have not been implicated in the development of BIA-ALCL.

If you have textured implants, no one is recommending removal of them unless symptoms develop. If you are considering changing the size of your implants or going from saline to silicone, you can take that opportunity to change to smooth implants, which is what Dr. McMahan has been using almost exclusively for over ten years.

Textured implants were created to reduce the risk of capsular contracture, which seems to be beneficial for implants placed on top of the muscle only. They can also help with avoiding malrotation of shaped breast prostheses.

Until further studies have been done, the FDA is not recommending removing textured implants from the market. Both major plastic surgery organizations, ASPS and ASAPS have developed task forces to evaluate BIA-ALCL studies and provide recommendations to their member surgeons. The term ‘gummy bear’ implants arose several years ago when the two largest breast implant companies, Allergan and Mentor, began to develop silicone implants with more viscosity; that is, a thicker gel than what has been used in the past. Breast implant experts refer to the thickness of the gel as ‘cohesiveness.’ The more cohesive the gel, the more the implant retains its shape regardless of whether or not the implant shell is intact.

The first silicone implants that were used for breast augmentation had a very thin gel and a very thin shell. As a result, they had a very liquid feel similar to thick oil. There were several problems with these implants including bleeding of the gel through the implant shell, rupture, rippling and capsular contracture.  The latter is caused by the scar tissue around the implant shrinking resulting in the implant getting compressed and hard. The vast majority of patients with these implants eventually developed severe capsular contractures, implant ruptures or both.

The FDA reapproved silicone gel breast implants in November 2006 for use in the general population for breast augmentation in women over the age of 22 and for breast reconstruction. These implants have thicker shells and a more cohesive gel than their predecessors and very little, if any, leakage of silicone through the shell. They are not as cohesive, however, as the next generation of breast implants that have been nicknamed ‘gummy bear’ implants.

These new highly cohesive implants are now approved by the FDA and are available for general use. They are firmer than standard silicone implants so the feel is not as natural. They are all shaped implants and are therefore textured to try and prevent turning inside of the breast pocket. For these reasons and the fact that they are considerably more expensive than standard silicone gel implants, most plastic surgeons do not use them much for breast enhancement surgery.

A tubular breast condition or constricted breasts as they are also known, is relatively common among women and can occur in a wide variety of severities. In this situation the areola is larger than normal and it bulges out more than it is supposed to as breast tissue has ‘herniated’ out into it. In addition, the base of the breast against the chest wall is smaller than normal and the distance from the crease under the breast to the bottom edge of the areola is shorter than what it should be. This is where the ‘constricted’ name gets its origin as the base of the breast appears to be tightened or constricted.

In some cases, the condition is very mild with just a little protrusion of the breast tissue into the areola or the breast has just a slightly shortened distance from the crease to the areola. In severe cases, the areola encompasses most of the surface of the breast with marked protrusion of breast tissue that almost looks like a bubble and the base of the breast is so constricted that it is no larger or even smaller than the size of the areola so that the breast projects out like a tube.

The treatment of tubular breasts consists of performing a periareolar mastopexy, making the areola considerably smaller and pushing the herniated breast tissue back into the breast. At the same time, the base of the breast tissue is scored or cut to allow it to spread out and widen the base of the breast. Implants are also an important part of correcting constricted breasts. An appropriate sized implant must be chosen to increase volume, which is typically needed, and to help widen the base of the breast. My preference is to always put the implant under the muscle and smooth, round implants are usually best.

Today’s emphasis on exercising and being fit is having an effect of the size of breast implants that women are getting. Breast augmentation is as popular as ever and continues to be the most popular cosmetic procedure performed by plastic surgeons, including Dr. McMahan.

However, women that are runners and who exercise regularly are tending to stay away from large breast implants that can get in the way of performing physical activities. Running, particularly long distance running, can be especially difficult with larger implants causing neck, back and shoulder pain; symptoms that cause women with naturally large breasts to have breast reduction surgery.

Those who already have large implants and have undergone a lifestyle change of losing weight and exercising, often come in to have smaller implants put in. To undergo a noticeable change in size, usually at least a 100cc change in implant size is necessary. It is surprising, however, that many patients who elect to significantly reduce the size of their implants, find that they don’t need to have a breast lift at the same time.

Others find that they have grown breast tissue later in life and feel that they are now too big with their implants. Those with saline implants can have the saline removed as a minor office procedure to help them determine whether they can go without implants or change to a smaller size.

The length of time that the implants have been in has a lot to do with the need for a lift when the implants are either removed or replaced with smaller ones. Those who have implants for a year or less and then change to smaller implants rarely need to have a lift done at the same time unless they had sagging to begin with.

It is not uncommon after breast implant surgery for one implant to ‘drop’ faster than the other one resulting in asymmetry in the position of the implants and upper pole of the breasts. In reality, it is not so much of a movement of the implant as it is a repositioning of the implant contents from the upper aspect of the breast to the lower aspect and the tightness of the muscle loosens over time after surgery.

In these cases, I find it helpful to differentially wrap the breasts with an Ace wrap wound all the way around the chest; above the breast with the ‘higher’ implant and below the breast with the ‘lower’ implant. Wearing this as much as possible and firm enough to feel some significant pressure without causing pain can significantly improve the symmetry quicker than letting the implant ‘drop’ on its own and is much for effective than massaging.

Patients commonly ask when the can get back to normal activity after breast implant surgery, including workouts. No matter how good you are feeling after surgery, it is beneficial to heed the advise of your surgeon and not overdue it for the first few weeks.

Most implants are placed under the pectoralis major muscle, which has been partially cut, elevated, sutured and is now stretched over a prosthesis. Elevating the elbow above the shoulder puts additional, unnecessary tension on it and can cause the sutures to tear out, resulting in bleeding or implant malposition. Therefore, it is recommended to keep the elbows below the shoulders for two to three weeks after breast implant surgery.

Strenuous activity too soon after surgery can also cause bleeding and other implant problems, which could result in the need to back to surgery. Give yourself adequate time to heal and you can avoid these problems and, eventually get back to all normal and strenuous activities.

I started with the “Ten biggest lies in plastic surgery,” but I now have to add #11. It’s the ‘Laser Bra.’ This is primarily a hoax to lure patients into having breast lift surgery by making them believe that a laser will give them a longer lasting result. There are two problems with this:  A. There is no evidence that the claim is true and, B. some surgeons don’t even use the laser or they use it for such a short period of time that it couldn’t make a difference even if it were effective. Beware of gimmicks and exorbitant claims. If it sound too good to be true, it is. As a general rule, if you see it on TV, do not believe it!

The vast majority of plastic surgery products and procedures seen on so-caller ‘Expert Doctor” shows are nothing but paid advertisements. Where are all of those ‘latest and greatest’ products from five years ago that were supposed to provide live-changing results? Gone. All gone. It was all hype; no substance and no results.

One of the risks of breast augmentation surgery is capsular contracture. This happens when the scar tissue that forms around the implants either becomes or shrinks so that it compresses the implants, making them hard. The idea behind massaging your breasts is to keep the scar capsule larger than the implants so that the capsule cannot shrink. I am not aware of and study that has proven this effective. Besides, by putting the implants under the muscle, every time you move your arms, you technically would be massaging your implants.

Sadly, I know of surgeons who tell their patients to massage their implants and then blame them for their contractures saying that they did not do it enough.
